Following are required forms for the Mountain View High School Swim Team:

Swimmers & Parents 

Dear Swimmers & Parents, 

Welcome to the 2011 Mountain View & Kuna High School Swim Team! 

Try-outs: We will be having team try-outs during the week of August 22-26 during our practice at the Meridian Pool from 8:00 – 9:00 am. All new and returning swimmers need to attend and are expected to swim the whole practice. We will be limited to 40 swimmers, so team acceptance will be based on times and stroke proficiency. We will notify swimmers on during the week as they are accepted. All swimmers will be expected to bring all completed registration paperwork with them to the first day of practice Monday, August 22nd at 8:00 am. – with the exception of the volunteer forms.  Those will be collected at the parent meeting on Monday August 29th at the Meridian pool at 6:00 pm

* All paperwork must be completed and turned in before swimmers are allowed to enter the water. All fees must be paid at this time as well.  

Practice: Daily practices begin Monday August 22nd.  During the week of Aug 22-26 we will be practicing from 8:00 – 9:00 pm Monday thru Friday. Beginning Monday August 29th we will move to our regular practice time of 6:00 – 7:30 pm Monday through Friday.  

Each swimmer is expected to attend every practice. If you are unable to attend a specific practice, you are expected to notify one of the coaches in advance, in writing. Each swimmer is allowed 5 excused absences, anything beyond that is expected to be made-up, or you will be ineligible to letter.  

Money: We are always trying to keep the costs of swimming manageable, and we understand the financial strain associated with the new school year registration. We have outlined on the registration paperwork how the fees are broken down. If you have any other questions regarding fees please let me know. The fees this year are as follows:  

Fees:

YMCA Meet Fees -     $40.00

Meridian Pool Fees -   $75.00

TVHSS Fees -            $25.00

Team Fees -               $35.00

Total Fees -                 $175.00 

Price includes a team cap, T-Shirt, Nampa Invitational fees, meet water/snack fee.  

Please make checks payable to: Mountain View High School Swim Team, and turn in with all completed forms at the first day of practice on Monday, August, 22.

Additionally: The Idaho High School Activities Association, Idaho Health Examination and Consent Form must be on file at Mountain View (all Freshmen and Juniors must have had a physical before 2011-2012 school year commences). All new students to the district must have physical on file as well. Sophomores and Seniors must fill out the health questionnaire. If you do not have one on file, please obtain a form from school and make an appointment with your physician immediately. Swimmers without proof of physical on file will not be allowed in the water—please provide Coach Shannon with a copy of physical or questionnaire.  

There will be a mandatory parent meeting on Monday, August 29th, 6:00 PM, at Meridian Pool. Please note this meeting is mandatory-all swimmers must be represented by an adult. Information will be handed out to the parents at this meeting.

There are several forms that will need to be filled out and turned in before you swimmer will be allowed in the water to practice. The forms are as follows:

1) Registration Form

2) Parent/Family Volunteer Form

3) Consent Form

4) Behavioral Contract

5) MT. View High School Interim Questionnaire and Consent Form or Physical  (Freshmen, Juniors, and new swimmers).

6) Mt. View High School Student Emergency Information Form

7) Parent Insurance Acknowledgement Form

8) Meridian Pool Liability Release Form

 Please note that your swimmer will not be allowed in the water until all the above forms are turned in and all fees are paid or an arrangement for payment of fees is made. All forms are available at www.mvhsswim.com

Volunteers: As always High School Swimming is a non-sanctioned sport, as such we rely on volunteers to run our meets. We will be making a volunteer schedule for the season for parents to sign-up. Each swimmers family is expected to volunteer at a minimum of 2 meets. Any swimmer without the pre-arranged 2 volunteer sessions will not be allowed to swim. 

Each Treasure Valley high school must provide 1 Building Marshall, 2 stroke and turn judges and 3 timers during each swim meet. This is mandatory – if we do not provide these officials, our team will forfeit that meet.

Please plan on attending the judge’s clinic on August 25th at West YMCA 6:30-8:30pm or August 29th at the West YMCA from 6:30-8:30pm. If we cannot provide judges at the meets, our swimmers will not be able to swim. During the clinics you will review all high school swimming stroke and turn rules. Anyone can learn from this clinic - no matter what your level of swimming experience is. Please inform Coach Isaac if you are NOT attending one of these clinics, as we need as many potential judges as possible.  

Our team’s swimsuit, although not required, is strongly encouraged and may be purchased at the Swim & Run Shop, 8620 Fairview Ave. in Boise. Make sure you mention “Mountain View Swim Team” to get the team discount (usually a 25%).
